If you move outside of our practice boundary but feel that it would be more beneficial for you to stay registered here - due to working in the area for example - you can now apply for an 'Out of Area' registration status.
Due to living outside of the catchment area, patients with an 'Out of Area' registration will not be eligible for home visits. In these instances, patients would have to contact 111 who will be able to advise of an alternative service.
Patients who wish to apply for an 'Out of Area' registration status, should update their address with the reception staff and ask for an Out of Area registration form. Applications will be discussed by the Partners in order to assess whether it is clinically appropriate for a patient to have this status.
Applications may be reviewed, on clinical grounds at any time to ensure that patients receive the most appropriate care.
